How to Complete the Entry Form: 

1. You must be a member of an Ontario B.A.S.S. Nation to participate in the Ontario B.A.S.S. Nation Qualifier.
2. If you are a member print off the entry form in the link above. It is a fillable PDF form.
3. YOU MUST complete the form entirely. Missing fields may result in your entry not being accepted.
4. Send your entry form and payment to our Ontario B.A.S.S. Nation Chapter.
5. Your Ontario B.A.S.S. Nation Chapter will sign your entry form as a member in good standing.
6. Your Ontario B.A.S.S. Nation Chapter will send your entry, as well as other club entries to the Ontario B.A.S.S. Nation President (contact information is on the entry form).
